Giving us an idea of your previous experience allows us to tailor the course to meet the needs of the different participants.

We have an obligation to ensure you are able to take part in the course, despite any physical or learning difficulty you might have. As long as you are able to perform the role of a phlebotomist, there should be no unfair barriers that stand in your way.

As well as helping us to understand your motivation, we use this short essay to assess if you might need extra assistance with the written assignment part of the course.

We ask these questions so that we can ensure that we are providing courses to a fair representation of the population in the UK, and creating professional phlebotomists with a wide range of backgrounds and experiences.

You do not have to answer these questions, but it will help us if you do.

It is important that we make you aware of certain policies and legislation regarding your course. Please take a moment to read through these documents, and declare that you have understood them.

We will archive the information in this form in secure storage for 5 years before destroying it. This information will not be passed onto any third parties, and will only be accessed by your Assessor and the PRO Phlebotomy Training Centre.
